Our intent is to describe for you who we are, what services we offer, and where you may go to get more information. We created this Web site for you as a convenience and for education purposes. Accessing and using this Web site does not create a physician-patient relationship.

Your accessing and using this Web site means that you accept and continuously agree to all of our Terms and Conditions, which we may periodically change and update. If you do not agree to these Terms and Conditions, you may not use this Web site.

Educational Information and Links-Not Medical Advice

You may use this Web site to access information about services we offer. You also may access links to other resources where you can find out more information that may be of interest to you. We are not affiliated with these links and are not responsible for the information appearing in them.

E-mail Use

E-mail is one way our patients may communicate with us for certain routine matters, such as:

  • Scheduling, confirming, changing, or canceling an appointment
  • Requesting a refill for your prescription
  • Billing and insurance questions
  • Asking for copies of your medical records
  • Letting us know about a change of address or telephone number
  • Informing us about a change in your insurance coverage
  • Requesting a referral

    Do not use e-mail to ask specific medical questions or to report medical problems. Instead, call us at 614/538-0527 or toll-free at 877/566-4311.

    Never e-mail us if you are having chest pains, or if you are experiencing any other emergency condition that you believe requires immediate medical care. Go to the nearest emergency department, or call 911.

    We have certain Rules and Responsibilities for using e-mail. This is for your protection, as well as for ours. We grant you permission to e-mail us, so long as you agree to follow these rules and responsibilities:

    1. We do not answer unsolicited e-mails from persons who are not our patients. If you would like to become a patient, please call us, or have your physician make the referral.

    2. You need to give us your e-mail address in advance, so we have a record of it. We may give you a password or use other means to help assure that we are communicating with you. If we receive a question or request from the e-mail address we have on record, we will assume it is from you and that your authorize us to respond by e-mail to that e-mail address. We cannot be responsible if someone else uses your e-mail address. If you are concerned that other persons who use your e-mail address may see information you do not want them to know about, please do not use e-mail. Call us instead.

    3. To help us direct your e-mail to the appropriate person, please use one of the following standard subject lines:

    • Appointment
    • Prescription
    • Billing and Insurance
    • Medical Records
    • Practice Administrator
    • Other
    Using a subject line other than one of these standard ones may delay our response and, in some instances, may result in no response at all.

    4. Do not use e-mail to ask medical questions or to report emergency conditions.

    5. Do not use e-mail to send us advertisements or promotional materials.

    6. Do not use profanity or abusive language.

    7. Sending information by e-mail may not be secure. Despite the precautions we take, the potential exists for e-mail to be improperly accessed and used by hackers. Information transmitted by e-mail is not encrypted. We cannot guarantee security. Nor can we be responsible for any improper access or use. By e-mailing us information, you agree to accept the risk that confidentiality or security may be breached. If you are concerned about confidentiality, please call us instead of using e-mail.

    8. When communicating with us by e-mail, please do not send attachments, unless you are returning an attachment we e-mailed to you, or you receive our permission to send an attachment. Attachments can introduce viruses. Our general policy is not to open attachments.

    9. Please do not send a "CC" of your e-mail to others. We will respond only to you and will not send a "CC" of our e-mail to others. If you are concerned about maintaining the confidentiality of our communication, be careful about forwarding any of our e-mail communications.

    10. One or more persons, who may not be physicians, will check e-mails. We only check e-mails during normal office hours. We do not check e-mails on weekends or holidays. We may not check e-mails each day. Usually, we will try to respond to an e-mail within two business days. If you do not receive a response, please call us.

    11. We reserve the right, with or without cause, at any time, to discontinue our permission for you to use e-mail or this Web site.

    Privacy

    Information we receive is subject to our Privacy Policy. Please click here or go to the heading "About Our Practice" to read our Notice of Privacy Practices.
